Once you’ve had discussions about your hopes, expectations and what is important to you, it is a good idea to record these in some way and share with the people who are close to you and health and social care professionals.

You could ask your GP or other health and social care professionals if they have special future care planning document or process you should follow.

Alternatively, you might find this My ACP document a useful way of structuring your thoughts and recording your preferences. 

Your thoughts and preferences may change over time, and if so, it is important to let people know.

By having these conversations and agreeing to have them written down you are giving guidance, confidence and strength to those closest to you in case you become ill and they have to speak or make decisions for you.
